Guest House Homeless Clinic
The Guest House Homeless Clinic Offers outpatient medication and mental health support services to adults experiencing homelessness and struggling with mental health challenges. Guest House provides consumers with access to mental health and supportive services.
Guest House supports their consumers by offering voluntary outpatient mental health support services, peer and group supports, medication and street outreach. Guest House also offers co-occurring groups and services to support individuals struggling with both mental health and addiction challenges. Staff work with consumers on their path to recovery by fostering wellness, optimism, and self-reliance.
Program Start Date: The Guest House Homeless Clinic first opened its doors for service in August 1991.
Connections Lounge is drop in center for anyone experiencing homelessness. The hours are 9-3pm Monday through Friday.
Laundry services, light refreshments and other community support are provided.
El Hogar’s Specialized Multiple Advocate Resource Team’s (SMART) mission is to provide services that contribute to the mental health and emotional well-being of individuals and families in the community. SMART assists Sacramento County residents, who are experiencing homelessness and struggling with a mental health disability, to connect SSI/ SSDI services which improves their health, housing and financial outcomes.

Funding Sources: This program is funded by the Sacramento County Division of Behavioral Health Services through the voter approved Proposition 63, Mental Health Services Act (MHSA).
Client fees: There is no charge for services at the Guest House Homeless Clinic.
Population: Adults (ages 18+) experiencing mental health challenges who are also experiencing homelessness (or at risk of homelessness).
Referral Sources: In order to ensure access to all, Guest House accepts new consumers on a walk in basis on Tuesdays and Wednesdays at 10:00am and Thursdays at 1pm.

Frequency of Services: Services are tailored to the individual needs and preferences of the consumer. The frequency of services also depends on the needs of the consumer. Guest House staff are available by phone or face-to-face as needed or requested by the client.

SMART has a unique partnership with local Social Security Administration and Disability Determination Services branches which helps with the expedited application process. SMART serves as a “one stop shop” where individuals can often complete an SSI application with a trained Benefits Specialist who can walk them through the entire process.
SMART is funded by the Sacramento County Department of Behavioral Health and Proposition 63, the Mental Health Services Act.
WALK INS ARE WELCOME ON FRIDAYS
To qualify for El Hogar’s SMART Program, applicants must have eligibility for, and/or be currently enrolled in an outpatient mental health treatment program and have all of the following:
1.) Age 64 and under
2.) Disabled and cannot work for 12 months
3.) US Citizen or Permanent Resident prior to 8/20/1996
Additionally, SMART applicants must:
1.) Be clean and sober at least 90 days
2.) Have medical history showing severity of medical and mental health conditions
