
The El Hogar Sierra
Elder Wellness Program began in May 2007
to address the need for specialized
outpatient mental health services that
fit the needs of the growing older adult
population (age 60+), as well as
transition age adults (age 55-59) in
Sacramento, California. The Sierra
Elder Wellness Program will have the
capacity to serve 174 seriously mentally
ill older adults, and will provide,
refer, or advocate for a full range of
services including:

The
Sierra Elder Wellness
Program’s
multi-disciplinary team
works collaboratively
with community partners
to access needed
medical/dental,
educational, financial,
cultural, housing, and
other community
services.
Community partners may
include landlords,
attorneys, payees,
Sacramento County
In-Home Supportive
Services Agency, other
home services, and
cultural healers as
agreed upon with the
treatment team, partner
and family.
Candidates currently
receiving services at
one of Sacramento’s four
Regional Support Teams
may qualify for
services.
