Program for Enhanced Community Care Information

The Program for Enhanced Community Care (PECC) includes an Enhanced Care Management Team and Community Health Workers who deliver a comprehensive approach to health care coordination. The team works to reduce barriers and ensure members are connected to all aspects of their health journey. Services are provided in a person-centered, strength-based, and trauma-informed manner, meeting members where they are within the community. PECC is committed to supporting members in addressing health-related concerns and achieving improved health outcomes in ways that align with their individual needs


You may qualify for an Enhanced Care Management or Community Health Worker program if you:

  • 18 years and older
  • Are enrolled in Medi-Cal through a managed care plan such as Anthem, Molina, or Healthnet
  • Meet one or more of the defined high-need criteria as defined below: 
  • Experiencing homelessness or unstable housing. 
  • At risk for avoidable emergency room or hospital use (high healthcare utilization). 
  • Have serious mental health conditions and/or substance use disorder needs. 
  • Transitioning from incarceration (jail or prison). 
  • At risk for long-term care institutionalization. 
  • Nursing facility residents who want to return to community living. 
  • Pregnant or postpartum individuals at risk for adverse perinatal outcomes 

Services Provided: 

Our Enhanced Care Management (ECM) and Community Health Worker (CHW) services provide comprehensive, whole-person support for individuals with complex medical, behavioral, and social needs. Through ECM, our care teams conduct thorough assessments, develop individualized care plans, coordinate medical and behavioral health services, and support members during critical transitions such as hospital discharge or reentry into the community. Our Community Health Workers extend this support by offering health education, care navigation, advocacy, and connections to essential community resources such as housing, food, and transportation. Together, these services reduce barriers to care, strengthen community connections, and empower individuals to improve their overall health and well-being.
