The Sierra Elder Wellness Program (SEWP) is a field program providing specialized outpatient mental health services for the growing older adult population (age 55+). Services include outreach, mental health rehabilitation and therapy, psychiatric medication and follow up services, housing support, case management, and much more.

This program is funded by the Sacramento County Division of Behavioral Services through the voter approved Proposition 63, Mental Health Services Act (MHSA).
Client Fees:
There is no charge for services at the Sierra Elder Wellness Program.
Population:
Older adults (ages 55+) living in Sacramento County who are experiencing functional impairment in important stages of their life due to chronic mental health symptoms.
Referral Sources:
SEWP accepts referrals only through the Sacramento County Adult Access Team.

Frequency of Services:
The frequency of services depends on the needs of the consumers. Many consumers receive various services, such as medication management, groups, and social rehabilitation on a weekly basis. In general, a consumer receives services at least once per week. Service frequency would decrease over time as the consumer prepares for graduation. Services are tailored to the individual needs and preferences of the consumer.
